If you have health insurance from other insurance companies, then: get services from us ->; submit a claim to your insurer with a copy of the receipt ->; get rewarded by transfer to your account.

For adults:

• identity document - passport or identification card( a driver's license is not an identity document! ) ;
• insurance policy( if there is one) ;
• the results of recent examinations and tests, if you have them;
• a referral from a family doctor or specialist if the payer is an insurance company.

Children under the age of 14 can visit a doctor only with an adult! If the doctor is visited with one of the parents, then it must be accompanied:

• for the child's birth certificate, which indicates the parent's name and the parent's passport or identification card or
• for the child's passport or identification card and the parent's passport with an entry about the child.

If the child visits the doctor together with another adult( sister, brother, grandparents or other relatives), then it should be with you:

• for a notarized power of attorney to represent the child's legal rights and interests;
• for an adult's passport or identification card.

Children from the age of 15 must be accompanied when visiting the doctor:

• passport or identification card.
